Prepare for Your Visit with Us

We strive to make understanding your treatment, total fee, and payment arrangements as stress-free as possible. Please bring your insurance plan information with you on your first visit. We will work with you to ensure that you receive the maximum benefits to which you are entitled.

We are happy to process your insurance claims to maximize your benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. As a service to our patients, we will bill insurance companies for services and allow them 45 days to render payment. After 45 days, you are responsible for the entire balance, paid-in-full. After reading about accepted insurance plans and our payment options, feel free to call our office with any questions, our courteous staff is always available to answer them.

Patients with Dental Insurance

We are glad you have dental insurance to help you in affording your dental care. Gupta Family Dentistry PLLC is in network with many insurance companies. We’re happy to submit dental claims for you and accept assignment of benefits to receive payment directly from your insurance company. Any remaining balance after your insurance has paid is your responsibility. If your insurance company is not listed below, please contact us to find out if we are in network with them. 


Out of Network :

We gladly accept most other dental insurance plans as out of network and are happy to file claims on your behalf. We ask for full payment at the time services are rendered and your insurance company will send reimbursement directly to you.

We will always do our best to help you to maximize your insurance benefits. However, your treatment plan is individually based on one’s diagnosis and is not based on your dental insurance benefits or lack of benefits.


We submit Electronic claim submission allows reimbursement typically in 2 weeks. It is your responsibility to follow up on claims submitted, and we will help you receive your full benefit should the insurance company require any additional information. 


Dental insurance is different than most medical insurance plans and it is important to be aware of the following:

  • Insurance is an agreement between you and your insurance company. The insurance relationship constitutes an agreement between the carrier, the employer, and the patient. Our dental office is not a party to that contract. As such, we can make no guarantee of the estimated coverage or payment.

  • Full dental fees are not always covered. Insurance companies base the amounts they pay on restrictive fee schedules, regardless of what our actual fee may be.

  • Not all dental services that are necessary for proper dental health are a covered benefit in all contracts. This depends on the kind of plan you or your employer has purchased.
